The shower scene was nicely done, though the whole time I was hoping it'd pan a bit lower. Just a smidge. But if that were the case, Jamie's tweet about his kids watching might've wound up quite different.

Re: HH as a bully - I'm starting to see him as a tough-love sort of parent to the doctors, of an extreme variety. He goes out of his way to take their egos down a notch as he grills them, which is infuriating at the time, but looking back at those moments you can see he's has a point that they can reflect on once they calm down afterwards, and keep in mind the next time they walk into the operating room. It doesn't excuse him - I think he goes too far at times - but I can understand where he's coming from, having been on the receiving end of such behavior in the past.

As for Ty/Tina and the look at the end, I was reading it as a moment when a man realizes there's a side to woman he's not seen before as she's holding a baby. Kinda cliche, but maybe it'll build to something later? I've not read the book, so we'll see.

Re: your Hooten comments, I agree. He does still push the envelope too far to the point where you want to throttle him. (Yoink that bow tie a little too hard... WHOOPS. ;) ) And it's a credit to Fred being the legend that he is that he isn't so infuriating I want to throw things at the TV! But he is absolutely a tough-love dad. (I still say Gato is the mom, too. Bwah.)
